Dementia Support at Home

Our Dementia Support at Home service helps people live safely and with dignity in familiar surroundings. We understand that dementia can affect memory, communication, confidence, mood and daily routines. We provide calm, patient and consistent support that helps reduce distress and maintain comfort.

We support people with personal care, meals, medication prompts, daily structure, reassurance, meaningful activity and family communication. We focus on what matters to the person, including familiar routines, life history, preferences, relationships and emotional wellbeing.

Care at Home & Personal Support

Our Care at Home and Personal Support service helps people remain safe, comfortable and independent in their own homes. We provide respectful support with daily routines such as washing, dressing, continence care, mobility, meals, medication prompts, wellbeing checks and personal presentation.

We take time to understand each person’s needs, choices, routines and personal outcomes. Our support is planned in a way that protects dignity, promotes independence and helps people feel listened to and valued.

Community Access & Meaningful Activity Support

Our Community Access and Meaningful Activity Support helps people stay connected to their local community. We support people to attend appointments, take part in activities, visit family or friends, access local services, build confidence and enjoy social opportunities.

We also help people develop skills, structure and routine. Our aim is to reduce isolation, promote inclusion and support each person to enjoy a meaningful daily life.

Palliative & End-of-Life Support

Our Palliative and End-of-Life Support is provided with compassion, dignity and sensitivity. We support people living with serious illness or approaching the final stages of life, helping them remain as comfortable and settled as possible at home.

Our role is to provide practical, personal and emotional support. This may include personal care, comfort support, meals and drinks, companionship, family support and assistance with daily routines. We work alongside families, GPs, district nurses, community nursing teams, hospices and other health professionals where they are involved.

Respite Support for Families & Unpaid Carers

Our Respite Support gives families and unpaid carers time to rest, attend appointments, manage other responsibilities or take a short break. We know that caring for someone can be rewarding, but it can also be tiring and emotionally demanding.

We provide safe, kind and reliable support for the person receiving care, while giving carers confidence that their loved one is being treated with dignity and compassion. Respite support can be arranged for short visits, longer periods or planned regular support, depending on assessed needs and availability.

Mental Health & Wellbeing Support

Our Mental Health and Wellbeing Support helps people build stability, confidence and structure in everyday life. We provide calm, respectful and confidential support that is shaped around each person’s needs, goals and recovery journey.

Support may include help with daily routines, appointments, medication prompts, social inclusion, emotional wellbeing, community access and practical daily living skills. Where appropriate, we work alongside families, social work teams, community mental health teams and other professionals.

Practical Home Support

Our Practical Home Support helps people maintain a clean, safe and comfortable home environment. This may include light domestic tasks, laundry, changing bedding, meal preparation, shopping support and help with everyday household routines.

We provide this support in a respectful way. We encourage independence where possible and work with the person rather than taking over tasks they can safely do for themselves.

Sensory Impairment Support

Our Sensory Impairment Support helps people with sight loss, hearing loss or dual sensory loss to remain safe, connected and independent. We adapt our support to each person’s communication needs, preferences and daily routines.

Support may include help with orientation, home safety, correspondence, appointments, community access, daily living tasks and maintaining social connections. We work with patience, respect and clear communication.

Physical Disability Support

Our Physical Disability Support helps people live as independently and safely as possible. We support people with personal care, mobility, moving and assisting, equipment use, meals, household routines, appointments and community access.

We focus on each person’s abilities, choices and aspirations. Our support is planned to reduce barriers, promote confidence and protect dignity.

Live-in and Residential Care

Our Live-in and Residential Care service provides safe, reliable and person-centred support for people who need a higher level of care, supervision or reassurance. We support people who wish to remain in familiar surroundings through live-in care, and we can also support people in residential settings where the correct registration, placement and care arrangements are in place.

Support may include personal care, meals, medication prompts, mobility support, wellbeing checks, companionship, household routines, community access and emotional support. We take time to understand each person’s needs, choices, routines and personal outcomes.

Our aim is to provide care that feels safe, respectful and consistent. We focus on dignity, independence, comfort and quality of life, while working with families, unpaid carers, social work teams, health professionals and other partners where appropriate.

Supported Living Support

Our Supported Living Support helps people live with greater independence, choice and confidence in their own tenancy, shared living setting or community-based accommodation. We do not believe support should take over a person’s life. We believe it should help each person build skills, confidence and control.

We support people with mental health needs, autism, learning disabilities, physical disabilities, sensory impairments, addiction recovery needs and other support needs. Each person receives support that is shaped around their strengths, communication needs, daily routines, risks, goals and personal outcomes.

Our support may include help with daily living skills, personal care, medication prompts, appointments, tenancy-related routines, community access, social inclusion and meaningful activity.

Bespoke Mental Health Rehabilitation & Step-down Support

Our Bespoke Mental Health Rehabilitation and Step-down Support helps people move towards greater stability, confidence and independence. This service is suitable for people who need structured support after hospital discharge, crisis, relapse, placement breakdown or a period of increased mental health need.

We provide person-centred support with daily routines, emotional wellbeing, medication prompts, appointments, community access, confidence-building, practical living skills and social inclusion. Support is shaped around each person’s recovery goals, strengths, risks, choices and personal outcomes.

We work with the person, their family, unpaid carers, social work teams, community mental health teams, health professionals and other partners where appropriate. Our aim is to help each person rebuild confidence, reduce the risk of crisis or readmission, and move forward safely at a pace that feels right for them.

Our Staffing & Workforce Support Services

Nurse Practitioners

We can support health and care services with suitably qualified and registered Nurse Practitioners where required. Nurse Practitioners bring advanced clinical knowledge and experience to appropriate settings, supporting assessment, care planning, treatment, clinical decision-making and service delivery within their professional scope of practice.

We take care to match each Nurse Practitioner to the needs of the placement. This includes checking registration, qualifications, experience, references and suitability for the role. Our aim is to provide skilled professionals who work safely, respectfully and in line with the requirements of the setting they are supporting.

Mental Health Nurses

Our Registered Mental Health Nurses support people with mental health needs in a respectful, recovery-focused and person-centred way. They help people build stability, confidence, independence and wellbeing, while working in line with care plans, clinical guidance and professional standards.

Mental Health Nurses may support people in hospitals, community settings, supported living services, residential settings or people’s own homes, depending on the placement and agreed role. They may work alongside GPs, psychiatrists, psychologists, social workers, occupational therapists, community mental health teams, support workers and families where appropriate.

Registered General Nurses

Our Registered General Nurses provide professional nursing support across suitable health and care settings. This may include hospitals, care homes, hospices, clinics, community settings, supported living environments and people’s own homes, depending on the role and service requirements.

We understand that every placement is different. We therefore match nurses carefully to the needs of each service, taking account of their registration, experience, skills, training and competency. Our nurses are expected to work with integrity, professionalism and respect, while supporting safe and effective care.

Healthcare Support Workers

Our Healthcare Support Workers provide practical, compassionate and reliable support in health and social care settings. They assist with daily care, comfort, observation, communication, mobility, meals, personal care and wellbeing, depending on the setting and agreed responsibilities.

We carefully vet and prepare our Healthcare Support Workers before placement. This includes appropriate checks, references, training and role matching. We look for staff who are kind, patient, respectful and able to support people safely under the right supervision.

Recruitment & Workforce Support

We provide recruitment and workforce support for health and social care services that need reliable staff. We work carefully to match the right person to the right role, taking account of skills, experience, values, availability and the needs of the service.

Our recruitment checks may include identity checks, right to work checks, Disclosure Scotland and PVG checks where required, references, qualification checks, registration checks and training records. We also consider each person’s attitude, communication style and suitability for working in care and support settings.

Support Staff

We can also support services with non-clinical staff who help health and care environments run safely and effectively. This may include administrators, domestic staff, catering staff, portering staff, IT support, finance support and other operational roles.

We understand that non-clinical staff play an important part in service quality. Their work helps clinical and care teams focus on the people they support. We therefore aim to provide dependable staff who are professional, reliable and suited to the needs of each setting.
